Taking an omega-3 fatty acid supplement in

did not have a benefit in preventing heart attacks,

There is little evidence to support the widespread use of so-called
“combination therapies” to lower cholesterol, according to experts from
the Ottawa Hospital Research Institute and the University of Ottawa.
After completing the most comprehensive review of clinical studies to
date, Ottawa stroke specialist Dr. Michael Sharma and colleagues found
that there is not enough evidence to show that combination therapies
work better in adults at high risk for coronary artery disease than
high dose statin therapy alone.
Sharma’s team examined 102 studies that tested combination therapies
involving ezetimibe, niacin, bile acid sequestrants and omega-3 fatty
acids.
“The message these findings give to patients is that for those who don’t
respond to a low dose of statin, it would make sense to try a higher
dose of statin before trying a combination therapy,” Sharma said in an
interview.
“If the high-dose statin does not work or is not well
tolerated, or if there are other special circumstances, a combination
therapy may be a good option, but until more research is done, this is
not recommended for most patients.”
Sharma is the medical director of the Regional Stroke Centre at the
Ottawa Hospital, a clinical investigator at the Ottawa Hospital Research
Institute, assistant professor of medicine at the University of Ottawa
and deputy director of the Canadian Stroke Network.
The results were published in Tuesday’s online edition of Annals of
Internal Medicine.
Statins are the most widely prescribed class of cholesterol-lowering
drugs, but they are increasingly being used at a higher dose, or in
combination with other types of drugs, for people who don’t respond to
the standard dose of statin, Sharma said.

This review was conducted with funding from the Agency for Healthcare
Research and Quality in the U.S. Sharma also noted that it is
particularly important to have this kind of independent analysis of
drugs that are heavily marketed.
It is estimated that more than 35 million Canadians and Americans are
prescribed cholesterol-lowering drugs every year. Lowering cholesterol
has been shown to decrease the risk of heart attack, stroke and other
cardiovascular conditions, Sharma said.
He said it’s not known how many Canadians are prescribed statin in
combination with another therapy in the hopes it will improve their
health. He said an estimated five million Canadians “need intensive
cholesterol lowering” because they are at high risk of vascular disease,
coronary artery disease, arteriosclerosis or suffer from a combination
of risk factors.
Sharma says the use of combination therapies to combat vascular disease
has been “heavily marketed,” especially in the U.S. “Unless you can
prove that it’s better than what you currently have, stay with what you
have.”
The review also found that taking an omega-3 fatty acid supplement in
addition to a statin did not have a benefit in preventing heart attacks,
stroke or death. “It actually raises the level of LDL, which is the bad
cholesterol,” says Sharma, who added, “You’re probably better off having
two meals of fish per week than you are taking a capsule that has
omega-3s.”
He said his team’s review of the current research showed that many
studies were too short, or incomplete, and he suggested more and better
designed research into cholesterol-lowering drugs is needed. |
